Soccer for Success in School

Please complete this form to gain access to the U.S. Soccer Foundation Learning Lab. Once in the Learning Lab, you will have access to two courses: Soccer for Success in School and Coach-Mentor Training: The Blueprint.

After completing either of these online trainings, you will gain access to the Soccer for Success in School curriculum, which is designed for K-12 physical education teachers to use during the school day. The curriculum is aligned to national physical education standards and infuses communication and connection skills into every lesson. The curriculum provides educators with a turnkey resource to get kids active while improving their physical and health literacy.

Insurance - By checking the box below, I affirm that my school/school district carries appropriate insurance which protects its students on a primary basis from injuries including sexual abuse and molestation, personal injury, property damage and bodily injury liability. Additionally, my school/school district meets the statutory requirements for Workers Compensation liability insurance, covering all employees involved in this program. Self-insurance, as required by law, is an adequate replacement for the above-mentioned insurance coverages.

Employee/Volunteer Background Checks - By checking the box below, I affirm that my school/school district has a criminal history background check process in place for its employees, and that all employees or volunteers must be cleared through that process prior to beginning work for the school/district.
